Public Service Award: Nomination form

All nominees must complete this form in its entirety to be considered for the MACPA Public Service Award.

The annual Public Service Award honors a CPA who has made a significant contribution to the community through dedicated public service. The honoree is selected based on leadership and initiative, impact on the community, time commitment to charitable and civic activities and the number of years the candidate has participated in public service.

Attachments, such as references and background articles, are welcome but cannot be submitted in lieu of the official application form. If additional pages are required to list all of the nominee's public service achievements, the form can be duplicated as needed. Please note that only public service activities should be described. Professional service related to accounting organizations is not considered in determining the award winner and, therefore, should not be included.

Completed nomination forms must be received by May 15.
